Top 10 Pain Killers — Antipyretic and Anti Inflammatory Drugs

Pain killers (analgesics) are medicine that we usually take when we feel pain in our body. In this article, I have listed Top 10 Pain killers, antipyretic and anti inflammatory drugs that are selected on the basis of effectiveness to relieve pain. Antipyretic drugs reduce fever while anti inflammatory drugs reduce inflammation.

Most of pain killers have more or less all these 3 properties of relieving pain, reducing inflammation and fever. But Paracetamol is one which has only 2 properties that is relieves pain and reduces fever– no anti inflammatory action.

So here we have Top 10 Pain killers:

Number 10 — Pain Killers
— Paracetamol

Reason at number 10, among Top 10 Pain killers, is due to its mild to moderate action for relieving pain.

Some of brand names of Paracetamol are Tylenol, Ofirmev, Midol, Panadol, Disprol etc.

Other name of Paracetamol is Acetaminophen. As aforementioned, Paracetamol has two effects on body. One is relieving pain and other is reducing fever.

It is considered as safest analgesic discovered till today. It is OTC (Over the Counter) medicine; can get from any pharmacy without prescription.

Indications:

If you are suffering from Headache, Fever, Ear Pain, Mild to Moderate Body Pain, Post Vaccine Fever, Malaise and Migraine you can take this medicine with or without Prescription.

Dose:

Adult Dose is 500mg-1000mg PO (per oral) up to 4 times a day. Maximum dose is 4000mg PO in a day.
Children dose is 10-15mg/kg/dose PO.

Contraindication:

Paracetamol is contraindicated in Hypersensitivity. It means if you are sensitive with paracetamol and undergoes severe allergic reactions / anaphylactic shock, then don’t take this medicine.

Side Effects:

The only major side effect is liver toxicity. Gastric disturbance may also occur.


Number 9 — Pain killers
— Paracetamol + Caffeine

Addition of caffeine with paracetamol makes it at number 9, among Top 10 Pain killers.

Some of brand names of Paracetamol + Caffeine are Excedrin Tension Headache, Panadol Extra etc.

Paracetamol with combination of caffeine is more effective pain killer than Paracetamol alone.

Due to its CNS stimulation effect it is used off label brain focusing and anti-sleep by students.

It is also OTC (Over the Counter) medicine that is you can get from any pharmacy without prescription.

Indications:

If you are suffering from Headache, Backache, Migraine, Neuralgia you can take this medicine with or without prescription.

Dose:

Paracetamol 1000mg + Caffeine 130mg (2 Tablets PO) up to 4 times a day.
Maximum dose is Paracetamol 4000mg + Caffeine 520mg (8 Tablets PO) a day.

Contraindication:

It is contraindicated in hypersensitivity due to any of its ingredients.

Side Effects:

Flushing and Tachycardia, CNS Stimulation, Skin Eruptions may occur.


Number 8 — Pain killers
— Mefenamic Acid

Reason at number 8, among Top 10 Pain killers, is due to its mild to moderate action for relieving pain.

Some of brand names of Mefenamic Acid are Ponstel, Ponstan. etc.

Mefenamic Acid is a NSAID (Non-Steroidal Anti-Inflammatory Drug).

Indications:

If you are suffering from Fever, Migraine, Post Operative Pain, Traumatic Pain, Osteoarthritis, Toothache, Inflammation and/or Generalized Body Pain, you can take this medicine as OTC (Over the counter: without Prescription).

Dose:

Adults: 250mg PO 6 hourly as needed.
Children: 25mg/kg PO 8 hourly as needed.

Contraindication:

In conditions like Asthma, Ulcer, Hypersensitivity due to mefenamic acid, don’t take this medicine.

Side Effects:

GI Bleeding, Diarrhea, Skin Rashes, Dizziness, Depression, Potential to Agranulocytosis, Thrombocytopenia, Leucopenia, Heart Failure.

This medicine falls in Pregnancy Category C. It means this medicine is less safer that category B and category A medicine.


Number 7 — Pain killers
— Aspirin

Reason at number 7, among Top 10 Pain killers, is due to its mild to moderate action for relieving pain and quick disintegration of aspirin in water.

Some of brand names of Aspirin are Aspirin, Ecotrin, Disprin etc.

Other name of Aspirin is Acetyl Salicylic Acid. Aspirin is a NSAID (Non-Steroidal Anti-Inflammatory Drug).

It is also Over the Counter drug; can be get without prescription from any pharmacy.

Indications:

If you are suffering from Headache, Fever, Backache, Toothache, Premenstrual cramps, Arthritis, you can take this medicine.

Dose:

Adult: 325mg-1000mg PO 6 hourly as needed.
Children: 50-75mg/kg PO 6 hourly as needed.

Contraindication:

Peptic Ulceration, Hypersensitivity, Hemophilia, Renal and Hepatic Impairment.

Side Effects:

GI Bleeding, Bronchospasm, Tinnitus, Vertigo. This medicine falls in Pregnancy Category C. It means this medicine is less safer that category B and category A medicine.

— Diclofenac Sodium/Potassium

Reason at number 6, among Top 10 Pain killers, is due to its mild to moderate action for relieving acute and postoperative pain.

Some of brand names of Diclofenac are Voltaren, Voltral, Caflam etc.

Diclofenac Sodium/Potassium is a NSAID (Non-Steroidal Anti-Inflammatory Drug).

Indications:

If you are suffering from Acute Pain, Post Operative Pain, Low Back Pain, Arthritis, Musculoskeletal Disorders, you can take this medicine as Over the counter.

Dose:

Adult: 75-150mg PO 12 hourly as needed.

Contraindication:

In conditions like Renal Impairment, Peptic Ulcer and Asthma, Diclofenac should not be used.

Diclofenac may cause Peptic Ulcer, Anaphylaxis, Blood Dyscrasias, Vertigo, Nausea, Vomiting, Anorexia and Anxiety.

This medicine falls in Pregnancy Category C. It means this medicine is less safer that category B and category A medicine.

— Ibuprufen

Some of brand names of Ibuprofen are Advil, Motrin, Brufen ect.

Ibuprofen is a NSAID (Non-Steroidal Anti-Inflammatory Drug).

Dose:

Adult: 200-600mg PO 8 hourly as needed.
Children: 5-10 mg/kg PO 8 hourly as needed.

Contraindication:

If someone is suffering from GI Bleeding, Hypersensitivity, Bronchospasm, Renal Failure and Hypertension don’t take this medicine.

Side Effects:

Ibuprofen may cause Thrombocytopenia, Peptic Ulceration, Renal Failure Hepatic Dysfunction, Seizures, Anioedema and Blood Dyscrasias. This medicine falls in Pregnancy Category C. It means this medicine is less safer that category B and category A medicine.


Number 4 — Pain killers
— Codeine + Paracetamol

Reason at number 4, among Top 10 Pain killers, is due to its mild to moderate action for relieving pain but stronger effects than Ibuprofen.

Some of brand names of Codeine + Paracetamol are Co-Codamol, Codogesic etc.

Codeine is narcotic analgesic. Narcotic Analgesics are that medicine which reduces pain effectively but they have addiction property.

Addiction property means there is an increase urge to take medicine if stop taking that medicine.

That is why all the narcotics are controlled drugs, should not be taken without prescription and must be dispensed by a qualified pharmacist.

Indications:

Mild to Moderate pain.

Dose:

Adult: 1-2 Tablets PO 6 hourly as recommended by Doctor or Pharmacist.
Children: Not recommended under 13 years.

Contraindication:

Respiratory Depression, Elevated Intracranial Pressure.

Side Effects:

Hallucinations, Respiratory Depression, Convulsions, Dry Mouth, Mood Changes, Sweating, Bradycardia.

— Nalbuphine

Reason at number 3, among Top 10 Pain killers, due to its moderate to severe action for relieving pain and stronger effects than Codeine.

Some of brand names of Nalbuphine are Nubain, Nalbin etc.

Indications:

Moderate to Severe Pain, Epilepsy, Post Operative Pain.

Dose:

Adult: 10-20mg PO 12 hourly as recommended by Doctor or Pharmacist.
Children: 0.1-0.15mg/kg PO 6 hourly.

Contraindication:

Hypersensitivity, Opioid Dependence, Respiratory Depression.

Side Effects:

Dependence, Urticaria, Hypotension, Dizziness, Headache, Dyspnea, Nausea, Vomiting.

— Tramadol

Reason at number 2, among Top 10 Pain killers, is due to its moderate to severe action for relieving pain and comparable effects with Nalbuphine.

Some of brand names of Tramadol are Ultram, Tonoflex, Tramal etc.

Physical Dependence, Seizures, Coma, Respiratory Depression, Bronchospasm, Angioedema. Its Pregnancy Category is C.

— Morphine

Reason at number 1, among Top 10 Pain killers, due to its severe action for relieving severe pain and stronger effects than Tramadol, Nalbuphine and other NSAIDs.

Some of brand names of Morphine are MS Contin, Roxanol, Morphine Sulphate.

Morphine is strong narcotic analgesic.

Indications:

If someone is suffering from Severe Acute Pain, All forms of Epilepsy, Myocardial Infarction and severe injury except head injury, can take this medicine only on the prescription of qualified doctor and in the supervision of qualified pharmacist.

Dose:

Adult: 5-15mg IM 4 hourly as recommended by Doctor or Pharmacist. 5-15mg IV Infusion load for half an hour.
Children: 0.1-0.2mg/kg IM 4 hourly as recommended by Doctor or Pharmacist.

Contraindication:

This medicine should not be taken in Elevated Intracranial Pressure, Respiratory Depression and Head Injury.

Side Effects:

Increases Dependency, Anaphylactic Shock, Respiratory Depression, Cyanosis, Tremor, Dysphoria, Constipation, Hallucination, Restlessness, Nausea, Vomiting, Hypotension.
